The easiest way to get this is to hide in the top right corner at the boss. Start up 5 min mode and play normally killing as little as possible and just moving past each wave when you can until you get to the boss then just move to the top right corner of screen and he can't touch you. The only thing I have found that can damage you in the top right corner of the screen is the one that shoots missils at you that look like pencils.
