Here is a link to a complete playthrough og the game on it's hardest difficulty with full commentary. It's a pretty simple 3rd person affair, but some sections can be tricky due to the one-shot-kill enemy habits.

Not the hardest of games to complete on the highest difficulty apart from a couple of sections.
1. When you have to download the information whilst holding off the enemies on the Siberia Mission.
2. The last helicopter battle on Burma. (Watch the online videos, they are pretty good so see what happens and when)
The best advice I can give is to trigger the spawn points and then get back and hide in cover. Providing the enemy is capable of getting to you they will almost always walk to you so you can end up with a pile of bodies and loads of ammo.
The music appeared to go a little quieter when I had cleared all the baddies in a section so that was my trigger to move on.
One other bit of advice is that if you get to the next checkpoint it'll move you on in the game, you don't have to kill everyone in the way. The best example of this is at the start of the Burma level. Once you get outside ... run all the way around the outside (behind the orange jeeps), get rid of the 3 guys who are standing at the bottom of the ladder and climb it. Keep low and the checkpoint is there. Saves killing about 8 or 9 bad guys who are pretty good shots.
