After you have completed the game on any Difficulty you will unlock 007 difficulty. As the name suggests this is hard but stay in cover and you should pass no problem.
(Update 24.2.2012) To make this difficulty harder, one shot to the back or chest kills you instantly like I said above stay in cover use blind fire where possible and if you need to aim while in cover make sure you're quick. Do not move from cover until every close enemy is history.
