When you are locked by a missile you will receive a warning. You cannot do anything. Flares are automatically deployed, at least at Realistic difficulty and below, and you will receive this achievement after you have successfully chaffed 15 enemy missiles. Playing on higher difficulty levels will make missile attacks more frequent. The avoidances are cumulative.
