THIS IS STORY RELATED AND CAN NOT BE MISSED!

You will unlock this achievement after you successfully kill Rocco. Continue through the game like normal. You will get mission called 'Cathouse' (also an achievement for completing the mission). After the cut scene and a small fire fight in the strip club. You will automatically start 'kill Rocco' mission. Make your way through the building slowly and make use of the cover around you to survive. Once you've killed all of Rocco's goons and made it to the top alone with Rocco, you will get a cut scene, let the cut scene play (or skip it) and the achievement will unlock.
