Pretty easy to get. Shortly into the first chapter a monster will tear your arm off in a cut scene. After you get control back your arm will appear. Next to you. Pick it up and walk into the room on the left to trigger some of the gray faceless. One hit should kill and trigger your 5 gamerscore
