You'll unlock this at the end of the game after beating General RAAM and having completed all acts and chapters on Casual difficulty. You can complete the chapters in any mode: single player, co-op or a mixture of both. You have to have credit for completing every chapter and act on the Casual difficulty for the achievement to unlock, although if you play through the game on a higher difficulty before playing Casual, this will still unlock since the difficulty achievements are stackable.
