Go ahead and start the game on Normal. This game my take some getting used to so be patient with it.
NOTE: I put weapon achievements on the bottom. I recommend you work on torture attacks while going through your playthrough.

You must find all the Resting Places, or the "treasure chests". Some of the resting places are somewhat hidden, making yor job harder. However, there are incredibly helpful videos on Youtube, made by hsypersakura2009.
NOTE: The videos are MISSING one resting place. In chapter 10, after verse 2, after setting up the wheel and turning the city, get on top of the archway on your way to the next spot. This archway is shown in the chapter 10 video at time 0:50.

After collecting all the chests on a level, quitting to the chapter menu should be enough; level completion is not required, as far as I know. I'm not sure if difficulty matters, but playing on Normal worked for me.

Normal Playthrough
As you play you will earn
Execute a Torture Attack.
Your Y and B Attack
Beating the prologue
Execute a Wicked Weave attack while using Dodge Offset.
A hair attack. Use YBY without losing the combo
Engage Witch Time successfully 10 times.
Engage Witch Time successfully 10 times consecutively.
Go into Witch Time 10 times in a row.
Dodge the ?????? during Chapter 2.
Make sure when the streetcar comes down the road you jump (A) and dodge (RT) it or you will miss this. You can go back a checkpoint if you miss it
Purchase three new techniques.
If you haven't by now buy 3 techniques at the store.
Defeat ????? on any difficulty.
Defeat the boss for this achievement
Complete Chapters 1 through 4 on Normal difficulty.
Your progress so far in the game.
Earn 10 Platinum Medals. Must be earned in 10 different battles.
By now you could have earned this. If you haven't it's all good as you have way more opportunities. If you beat the game without this then try very easy mode as platinum medals are easy as hell.
Defend ????? during the ???????, ensuring ????? takes no damage.
Defend Cereza without her getting hit. Just get in the enemies faces with a combo and they don't really go after the girl
Defeat ????? on any difficulty.
Another big boss fight
Complete Chapters 5 through 7 on Normal difficulty.
Your progress so far.
Destroy all the tentacles that drop down together during in single sequence in Chapter 9.
When falling through a pit of tentacles land on the red spots or shoot them. Shooting reduces falling speed so that may be your best bet. Try not to miss this one. Go back a checkpoint if you do.

Whenever you want to buy an item or a technique the game has a "try before you buy option". Earn all these achievements there...
Kill an enemy by jumping on top of them.
Taunt and defeat five plus angered enemies while taking no damage. Gaze of Despair may be equipped.
Avert 10 enemy attacks with the Moon of Mahaa-Kalaa equipped.
Counter 10 enemy attacks with the Moon of Mahaa-Kalaa equipped.
Counter three enemy attacks consecutively with the Moon of Mahaa-Kalaa equipped.
????? 20 enemies while wearing ?????
Kill 20 enemies using only Iai-Jutsu with Shuraba. (Iai-Jutsu performed by holding the Y button.)
You want to earn this during the 5th or 10th Alfheim portal
While never setting foot on the ground, grab enemies 10 times using Kulshedra.
these stages basically require you to do this achievement anyways.
Try to earn this as you start or halfway through hard mode.
