I do not have this achievement but I do have Extremely Diligent (any hero completed 40% of all side quests).
For the character that caused the Extremely Diligent achievement to pop, I only ever played a campaign online with johncenajr16. He didn't receive the achievement because I was hosting but that was to be expected.
I can't speculate as to why this achievement is buggy but it definitely is.

Yes. This is why:
Only the host can get the achievement.
On top of that, if any player other than the host activates and completes quest 100 (20%) and/or 200 (40%) then the host will not get the achievement but the counter will still be at 100 or 200, thus making the achievement unobtainable for the host. The host must then create an entirely new character and start from 0 quests UNLESS they catch that this situation has happened and dashboard immediately (thus not saving progress). |

The host is the only one who gets the achievement by design, not by bug. A poor choice, but eh.
But you are right about the buggy flags, both have it now, thanks. |

| It's annoying because the way multi-player works in this game is amazing. The achievements being "host only" is the only drawback. |

| Found it rarely easy solo to complete 400 side quests, they are everywhere and requires very little effort. |
