Depending on your region, it'll be called either The Witcher 3: Wild Hunt Game of the Year Edition or The Witcher 3: Wild Hunt Complete Edition and will include both expansions, Hearts of Stone and Blood and Wine, as well as the miscellaneous DLC on the Marketplace such as extra quests and New Game + mode.Though the miscellaneous DLC was all free, the season pass is valued at US$24.99.
