Just want to add to this already good guide. To a point, Ghosts do count. I think it might be a certain % of ghosts killed because when my achievement popped, I had just killed 4 or 5 ghosts. I was at the end of the game where there are no more soldiers, just ghosts.
GsA UnReaLiZe ghosts do count when u actually making progress in the campaign, not by reloading a chapter because then it wont save your data.
