After you beat the game for the first time, you will unlock Insane difficulty. Don't let the name fool you, it really isn't all that hard. You can basically play exactly the same way you did on the easier difficulties. Just note you should probably do most of the side quests again to gain enough experience, because there will be points where you may not be leveled up enough to easily progress through the game. The only hard parts are some of the bosses, such as the one at the Opera Championships, and that crocodile guy at Hel's Half Acre, right before fighting Anti-Spank. For these guys, you should either run away and use lots of grenades or use the orb of invulnerability and start chaining your attacks (which means alternating which weapons you're hitting them with, and not hitting them with the same weapon twice in a row). Anti-Spank becomes especially easy if you stick an orb of invulnerability in front of each arm and his torso and start chaining attacks.

As well, be prepared to die a lot. Luckily, there isn't much of a downside to dying, besides wasting a little time. The enemies always stay dead if you kill them, though they will respawn if you leave the area and come back 10 or so minutes later. Just reflect as many projectiles as you can, and shield bash to stun guys, making life a little easier. I also found using grenades are great for groups of enemies, so that they won't suddenly ambush you and kill you in 5 seconds. Also, every time you respawn after dying, eat the most helpful food, as that will usually put you back to full health.

As well, every time you level up, you will get a choice of hero cards. I would advise focusing on the health increase/resistance or the stun/reflect hero card for the first set of 3, and the damage or the armor/weapons upgrade one, as these will be the most helpful to you during your Insane playthrough, especially the health one.

Krishna 93 Dunno if you want to but you should also mention about the hero cards cause i found it easy once i had the health increase melee damage increases as well as the stun and reflected damage card. This helps alot with the reflects and shield bashes. Otherwise good guide!

Alderon For the Hero Cards there's 1. Health/Resistance, 2. Damage(Melee/Ranged), 3. Running/Eating Speed, 4. Sidekick Damage/Health, 5. Reflection Damage/Stun Duration and 6. Using Armor/Weapons at Lower Levels.
