Simply rescue or harvest EVERY Little Sister in the game. A combination of rescuing and harvesting Little Sisters is permissible, however, if you harvest any Sisters, you pass up the opportunity to obtain the "Savior" achievement, on that playthrough, anyway.
When the game is paused, at the bottom of the screen you will see some icons. A picture of a Big Daddy and a Little Sister means either there are still Sisters to take from the Big daddies, or you are carrying a Little Sister.
When those pictures change into just a Little Sister, that means they have been dealt with.
BigBjoern So i need to decide to harvest every little sister or rescue every?
So a mix of rescueing and harvesting is not possible?

TheOtherBarber ^ I'm wondering the same thing, I'm not very far into the game yet and I've already saved my first little sister and since there's no gain from saving them (maybe, don't know yet) I'd rather just have the ADAM so I could buy upgrades to make the game go as fast as possible.

DrJekl If you rescue them all, you get an additional achievement, you can choose Harvest some and Rescue some though.
