As the description says, just complete episode 3 on "Hurt Me Plenty" difficulty or higher. While "Hurt Me Plenty" may be the easier option, completing it on "UltraViolent" will help towards the Rampage achievement, and will save you having to play through the episode twice. Please note this achievement cannot be obtained in co-op, it has to be done in single player.

If you do decide to play through it on "UltraViolent", remember that saving the game often can help, especially if you don't know what you're doing. This is where the game really starts the pick up in terms of difficulty, so remember to save every so often. There are a lot of nasty surprises hiding round corners, so it only takes being caught off guard once to get wiped out. Finding all the secrets in levels can really help you out too, as they often contain extra ammo and health/armour boosts to put you above 100%.

The beginning of the episode is one of the most challenging parts, due to the lack of ammo in the first few levels. But if you conserve it well enough, you should get by just fine.

Thanks to DaveOwl, I can confirm that you don't have to complete the secret level (Warrens) to get this achievement. And thanks to deutschZuid, I can also confirm that you don't need to complete this level to unlock the Rampage achievement. If you still want to complete it though, you can get to it from E3M6, Mt. Erebus. Watch the YouTube video to see how to get to the secret exit, as it is kinda hard to explain.

The secret level itself is almost identical to the first level of the episode, Hell Keep, and you only see the differences when you reach what was the end of E3M1. From here, you have to work your way back to the beginning of the level while fighting various monsters, including Cacodemons, Barons and a Cyberdemon. It's not all bad though, as there is a fair amount of ammo lying about, as well as some invulnerability spheres, so you should be able to skip by most of them if you so wish.

In the final level in the episode, Dis, the BFG can win the fight against the Spider Mastermind almost instantly. So if you have the ammo, just keep firing the BFG at it until it dies.
