As the description says, just complete episode 2 on "Hurt Me Plenty" difficulty or higher. While "Hurt Me Plenty" may be the easier option, completing it on "UltraViolent" will help towards the Rampage achievement, and will save you having to play through the episode twice. Please note this achievement cannot be obtained in co-op, it has to be done in single player.

If you do decide to play through it on "UltraViolent", remember that saving the game often can help, especially if you don't know what you're doing. It really isn't overly difficult, as long as you remember to conserve your ammo, and make sure you take your time and don't rush head first into fights.

Thanks to DaveOwl, I can confirm that you don't have to complete the secret level (Fortress of Mystery) to get this achievement. And thanks to deutschZuid, I can also confirm that you don't need to complete this level unlock the Rampage achievement. But as the level is quite easy, you may want to do it anyway, to get some extra ammo and health. You can access it from E2M5, Command Center (check the YouTube video to see how to find it). The level itself is really simple, and can be completed with ease by making the enemies infight with each other. Once all the monsters are dead, collect all the weapons and health, and exit the level.

In the final level in the episode, Tower Of Babel, don't be afraid to bring out the big guns, as you lose them after this level anyway. The Rocket Launcher is probably your best bet, as there is more than enough ammo for it lying about. But the Plasma Rifle, if you have enough ammo, is a great choice to, as it will dispatch the Cyberdemon in half the time the Rocket Launcher will.
