The first level of Episode 3(Inferno) is much easier to finish on nightmare than the first level of Episode 1(Knee Deep in the Dead). You do not need to unlock it to play it; it is available right from the start. This video guide shows you how to beat the level on nightmare while also getting 100% kills, items, and secrets very easily.

Some extra tips to complete E3M1:
- In the starting area, open the first door and lure the Cacodemons out into the open. Get them away from the door and preferably fighting with the Imps then run to the next area. Go through the doorway and run up to the shotgun bridge but stop before stepping onto it. Save your game here and reload. This resets the live enemies on the map so instead of chasing after you they will be chilling wherever they were when you saved and won't attack again until they re-notice you. Ideally this will leave them all in the big outdoor starting room, now unaware that you exist.
- At around 0:28 in the video I open a door to a room with some Imps and Cacodemons, then I run away and get the shotgun. I actually recommend NOT opening that door until you have the shotgun and are ready to proceed to the Pinky hall. That way you won't go back and open the door to find the Cacodemon right in your face. (The logic behind opening it early and running away is that hopefully the Cacodemon starts attacking the Imps and takes them out for you, saving you time if you are going for 100% kills)
- The bridge to the shotgun disappears as you step on it, so you need to run across straight into the next room. There are 3 Imps in there so run in guns blazing.
- The rest is pretty self explanatory. Shotgun all the Pinkies in the narrow hallway. Then for the Imps in the last room, just hide behind the wall, pop out for a shot, then jump back behind the wall. Rinse and repeat until all the Imps are killed. Then hurry through before they start respawning.

NOTE: This achievement cannot be unlocked in Co-op mode.
