In the mission "Corporate Collapse" , advance through and fight off the Ceph aliens til you reach the parking garage. Hargreave will tell you that you'll have to get in through the garage on the lobby level, blow up the door and swim to the next area and the achievement will unlock. This is a storyline achievement, and ergo cannot be missed.
