This achievement can glitch on you!

There are 7 Demon Doors (DD) you need to open. However, if you open the Ravenscar Keep DD last, your counter can reset, forcing you to replay the game with a fresh hero.There is no known way to remedy the glitch once it has happened, as you need to open the DDs in your game, not another hero's game.

So to avoid a second playthrough, first finish the main game and the DLC WITHOUT opening any DDs. Once you're done with those, commence opening the DDs, starting with the Ravenscar Keep DD, and then open the six others. The last one I opened was the Millfield DD, but I cannot confirm if this is necessary for the achievement to pop. It worked for me, though. It might also unlock if you open a any other door(s) after the DLC door (which worked on the 360 for me), but that is not guaranteed.

The requirements for opening each Demon Door:

Ravenscar Keep - The DLC Demon Door you should open first
Requires you to wear a full Prisoner's Outfit. Consists of 5 pieces, all found in and around Ravenscar Keep.
- Bandana: In a chest in the farm area, behind a wheelbarrow.
- Top: Only available after completing the main quest. Enter the keep, take the second door to the left and open the second cell to the left. You need the key from the Prison Management for Beginners side quest.
- Trousers: In a chest in the cemetary, where you kill the third group of Hollow Men during the Undeath Sentence quest.
- Boots: In a chest in the Gallows area, where the fence has broken down.
- Shackles: On a cliff in the beach area. You need to vault down to get to the chest.

Brightwall Village
You need to take one of your children by the hand and lead it to the door. So either adopt one or make one.

Mistpeak Valley
You need a co-op partner for this. Stand in front of the Demon Door and perform nice expressions until it opens. It only counts if the host performs.

Wear a pyjama, and dye each piece in a different colour. Remove all tattoos. Then keep eating pies until you're as fat as you can get. You can check your weight progress in the sanctuary.

City of Aurora
You have to be either 100% good, or 100% evil. I mixed in a good deed when I was evil, and one bad when I was good, and still managed to get it open.

You need a fully upgraded weapon and must have mastered (level 5) all three combat disciplines.

Sunset House
You need to be the crowned sovereign of Albion.
