Your combo meter increases with each consecutive hit. If you get hit by an enemy the counter is reset at once. The good thing is that if too much time passes by between hits the meter will only start to decrease and not reset. Nevertheless, the problem is to get enough enemies together for 100 consecutive hits.

At the end of Chaper 2 - Mission 6 you can use a trick to unlock the combo achievements relatively easy. When you fight the boss in the nightmare world he can be hit, but not hurt. He only takes damage once you purify all the summoned souls and the fight switches back to the real world. What you should do is to clear all but one blue soul while in the nightmare world (preferably one far from the boss), then stun the boss and you can hit him as many times as you want afterwards. Use double swords and the combo XXYY, even if the swords break.

Another opportunity to get this achievement is during Act 3 Mission 6 "The Imperial Gardens of Blachernae"

Late in the level, you are forced into Hell to fight off an endless horde of souls emanating from the two pits under the domed structures. You're meant to break the pillars to progress but instead make use of the unlimited supply of enemies and the ability to stay in Hell without the risk of losing health.

I worked my way round one structure in a circular pattern using the LT + A fire attack to take out each soul. They tend to spawn just in front of you if you keep going round and round. After a few passes the souls from the other dome should make their way to you and you can add them to your combo.

Make sure to stay away from spawning souls as a single hit will reduce your combo to 0. You may also have your combo drain by 5 or so if you don't get a hit in a second or two but this really isn't a problem as eventually you'll be almost overwhelmed with souls to take out. It also helps if your AI partner (or a co-op friend) stays away from your section of the level.

This is a slower method than the others but it's also the safer option as it's very hard to be hurt, stunned or miss and lose your combo. If you find the boss battle too hard to get the combo then hopefully this will help
