Spoiler free :) To get the Perfect Knight - Day 2 achievement, you'll need to have done the following:

*Beat the main game on either NORMAL or HARD. You cannot unlock New Game Plus on Easy so I don't think you can even get this achievement. Beating the main game shouldn't be a problem, even on Hard. you'll have beaten the story when you get the Exit Stage Right achievement.
Batman: Arkham CityExit Stage RightThe Exit Stage Right achievement in Batman: Arkham City worth 61 pointsAll the world is a stage

*Now that we have beaten the main story, we have unlocked a mode called New Game Plus (NG+) this is a game mode where you'll restart the story and in which you'll need to beat it again. still, this should not bring too many pain-in-the-ass moments. once you've beaten NG+ you will get the Twice Nightly achievement.

*Side missions. There's a lot of confusion about these. That has a good reason thus I will split it up in three parts.

The straight forward ones. These are the regular side-missions, which include AR-training, the detective missions, etc.. You will get an achievement for everyone you beat and you WILL need to beat every single side mission on NG+ as well

The Riddler's collectibles, upgrades, art concepts and character trophies. You will need to collect all Riddler trophies in EITHER regular game or NG+. but these DO carry over, so that's some good news. every riddle or trophie only needs to be collected once. After having collected enough trophies, one of the Riddler's hostages' locations will be shown to complete. you DO need to save every hostage one both regular game and NG+. during your search for these collectibles you'll get enough XP to get all your upgrades, every story, art concept, character trophies, you name it. you will get the Genius achievement after saving all hostages.

    EDIT 19-01-12:
Credit goes to NinjaDave04 for confirming this. You will also have to complete all the physical challenges (found on the same menu as The Riddler's trophies. these were not too hard for me even after beating the main story. If you have a question about a physical challenge, just leave a comment.

the most irritating things in this game I found to be were the random acts of violence side missions. these are random events, where a political prisoner is attacked by a thug and you will need to fullfill your task as the Dark Knight and save him. the annoying thing is that you, obviously, have to complete all 16 events twice (in regular game and NG+) you will find most of them during your hunt for riddles, but i ran out of things to do in NG+ and I was left with only these things to do. a proper and not-so-boring way to complete them is to fly around the city, complete a event, save and go back to the main menu. then load up your game again and you'll find it much easier to find all these events. repeat until you're done. i completed 2 events in 30 minutes without this technique, and 6 in 12 minutes with.
Batman: Arkham CityAggravated AssaultThe Aggravated Assault achievement in Batman: Arkham City worth 18 pointsStop all assaults in Arkham City

*Ranked Challenges. these are the regular Predator and Combat challenges you'll be familiar with if you played Batman: Arkham Asylum. they made the combat challenges a little easier, as it's not as hard to get the high combos, which are the key to getting a lot of points. but the Predator challenges are a lot harder this time. Luckily you are on Trueachievements right now and someone's posted some excellent videos of how to beat both Predator and Combat challenges. you will find them in the link of the Gold Revenge achievement
Batman: Arkham CityGold RevengeThe Gold Revenge achievement in Batman: Arkham City worth 178 pointsObtain all 72 medals on the original Arkham City ranked maps (as Batman)

*Campaign Challenges. these are new in the series. the name "campaign" is false. campaign is just three ranked challenges with choseable modifiers, that will either make your life easier or harder. i once again refer to the amazing videos that you will find in the link of the Gold Revenge achievement. you get 3 retries in one campaign challenge, so i would reccommend getting used to the challenges first in ranked, before going for the Campaign Gold achievement.
Batman: Arkham CityCampaign GoldThe Campaign Gold achievement in Batman: Arkham City worth 206 pointsObtain all 108 medals on the original Arkham City campaigns (as Batman)

If you did everything above, you should get the achievement. If you don't know what you missed, you can always go to the tracker list in the main menu that tells you exactly what you still need to do.

If you liked my solution, please give it a thumb up. It'll let me know if I'm helping people. If you didnt, leave a comment why.

Warboy925Yes I did, if you read the guide, I told the reader to do the first 4 hostages one after the other, and yes the riddles carry over....but you still have to redo the hostages

And what did you mean by "physical challenges"? If you are talking about all the gold medals, I believe it carries over to NG+ as well!

mudder85Just getting back to finish this game, can you go back after you beat the game on ng+ to do side missions or do you have to beat them all before beating the game again?

OrkMasterMcGeeAll right guys I need some help. I just finished up new game plus. I have 100% main story on both files. I finished all side missions on both files 100% yet, I’m still at a 78% on new game plus??? Riddlers revenge is fully complete, I have absolutely no idea what happened. Any ideas?? Hopefully my save didn’t glitch...

PrimeBigTimeOld game but I'm trying to finish this. Two questions that really aren't answered somewhere for some reason:
    1. After I finish the game on normal, does it go to free roam so I can explore the city and do all the riddler stuff and side missions or is the game completely done and I'm screwed?
    2. Is New Game + on a separate save file so I can jump between the two any time I want?

    CastMango150644@PrimeBigTime,
    1. After you beat the game, you will still be able to complete the side missions, free roam normal. Just won't me any main missions to do.
    2. The NG+ is part of your selected fille, 'cause you need to complete the game on normal or hard and then complete the NG+ in order to complete 100% of all (you will need to complete other stuff to have that percentage). What you will do in the NG+ won't affect your progresse in the normal/hard original run, but you will carry all your equipment e upgrades to the NG+. You can switch between NG+ and your original run normally in the main menu.

RCone138So for NG+ can we rescue the hostages right away or are they locked by story missions? Same with the side missions will all aggravated assaults be available from the start or locked behind story missions? Also do we do the physical riddler challenges twice? Cause that Doesnt make since. Does that mean we have to do Catwomans challenges twice too? Left a lot unanswered in this guide. Pretty disappointing

Bazingator@RCone138 Play the game and find out. When you are able to do the side missions is not relevant to the guide. Also, the guide clearly says you do not have to do riddler trophies twice.

Octobot SuperIs there a good text & picture based guide for the Riddler trophies? Preferably one that isn't some bloated crappy site like IGN?

PrimeBigTimeIf you're still looking for a picture guide, this one was pretty much perfect for me:
