Besides the fact that all the other guides for this achievement are for how to solve the "glitch" and not basically to get the achievement itself.

I will do a little briefing here: [With Some Spoilers]

To obtain this achievement you had to became a Companion Member (If you're looking for this achievement you must being a Companion by now, then you had to make some quests, became a Werewolf, found the Silver Hand, then:

-Aela asks you to search for the plans of some Silver Hand's members.
-When you came back, you ask she for a new job, she will tell you to go with the old man.
-The old man will ask you to take the witch head (Blood's Honor Quest)
-When you came back you will notice the Silver Hand attacked Jorrvaskr and Valkas will ask you to take some revenge (Purity of Revenge Quest) and bring back some fragments stolen.

When you came back (again to Jorrvaskr) you will start the Glory of the Dead Quest:
    -Attend Kodlak's Funeral
-Give the fragments to Eorlund (the blacksmith of skyforge).
-Retrieve Kodlak's Fragment (from his room)
-Give the last fragment to Eorlund (the blacksmith of skyforge).
-Go to the Underforge (who is Under the Forge, kindo)
-Then go to Ysgramor's Tomb. (around Winterhold area)
-As usual clear the tomb from ghosts and creatures.
    -Speak with Kodlak Ghost
    -Kill wolf spirit
    -Speak with Kodlak Ghost (again).

How to avoid the glitch that will prevent you from getting this quest.

    - preferable method- Don't ask the Jarl in Winterhold about any work until you have beaten the Companions quest line. He will give you a quest to find the Helm of Winterhold which interferes with the 2nd to last quest in the companions quest line.

    - if the quest is active and not yet complete - retrieve the Helm of Winterhold without killing the last 3 guys in the dungeon. I froze them with my dragon shout. Then turn in the helm before returning to the companions with the witches head.

    Hope this helps.

Disturbed XBAGot to go back to an older save myself. Got one of the Trouble in Skyrim quests to kill a leader who is already dead... This seriously needs sorting. Happens all across the game.

GroustaI just got the achievement yesterday after the game glitched on me. I cleared the area with the witches and took a head to the Companions. Then the Purity of Revenge quest did not start, because the Helm of Winterhold quest was active. I didn't realise it was a glitch back then. I complete the Helm of Winterhold quest, but nothing changed (I had no old saves). I just thought: this achievement is no longer obtainable, so I went on with the rest on the game. Yesterday (after a very long ingame time) I walked into Jorrvasker and the Purity of Revenge quest started!

I don't know if I was lucky or that it works for everyone, but I was very relieved. One think I did after I realised it was a glitch, is stop the mourning of the Companions by attacking one of them. Maybe this helped too, but this was done a long ingame time ago, so it does not help immediately.

Note: The "Helm of Winterhold" is a separate glitch but both can occur in your game, causing this achievement to be unobtainable. This guide deals with the glitch that occurs when you have already cleared Driftshade Refuge prior to completing the Companion Questline.

Regarding the glitch in between the Blood's Honor and Purity of Revenge Companion Quests, I have found a solution to avoid having to start a second character just to finish the Companion questline and get the Glory of the Dead achievement.

When you are tasked with going to Glenmoril Coven (in the Blood's Honor Quest) to obtain a Witches Head, do not go inside the Coven if you have already Cleared (check your in game map to verify) Driftshade Refuge. Driftshade Refuge is located a little southeast of Dawnstar.

If you have already cleared Driftshade Refuge, you will be able to finish Blood's Honor by talking to Vilkas, but he will not give you the next quest in the companions questline, which is Purity of Revenge. To fix this simply wait outside of the Glenmoril Coven for 32 in game days (24 hours at a time) and then proceed to finish the quest Blood's Honor as normal, clearing our Glenmoril Coven and then traveling to Jorrvaskr to speak to Vilkas. Once you speak to him, the glitch will not occur and Purity of Revenge will start and update in your quest journal as normal.

XanothusIn theory, they should work. 30-32 in game days seems to be the reset period where dungeons/areas reset themselves. It's worth a try, but since I never had that glitch occur in my multiple playthroughs, I can't confirm it 100%.

JiannerThe advice of 32 days worked for me. I finished Blood's Honor but Purity of Revenge didn't start. I then completed the Helm of Winterhold side-quest without killing the guys in the final room and then went to talk to Vilkas - but the Purity of Revenge quest still didn't start. I then waited 32 in-game days in Vilkas's room (kinda creepy I know) and talked to him again - still no luck. I then exited Jorrvaskr and the quest suddenly started.
