Ubisoft provide no documentation on the workings of the game so for those wondering how it works...

To earn this achievement you must collect 23 medals.

There are three sections: Activities, Workouts, and Classes. Under each section are types of exercise, then each type of exercise has sessions varying lengths/intensities. For example, under the Classes section you will find Boot Camp which has 6 sessions of increasing difficulty in terms of length and demands.

Whenever you complete any session for a particular exercise you will earn 1/8 of a medal and the difficulty of the session is not important. So you could simply complete the easiest session for each type of exercise 8 times to earn each of the 23 medals. However, it makes sense to combine earning this achievement with Meritorious (earn every badge), which means you will complete every session for every type of exercise. 16 of the 23 types of exercise have at least 8 sessions, so in the process or working towards the Meritorious achievement you will earn a medal also. However, exercises like Health [Humana] only has 3 x 13 minute sessions, so some repetition would be required.

As with the Meritorious achievement and for reference I have provided a complete list of all the exercises, with the number of sessions for that exercise in brackets. Note that you need Uplay points to unlock three of the sessions. However, as you play the game you will be awarded Uplay points for earning the following achievements:
Jump Around (complete any jump rope activity) - 10 Uplay points
Safari, So Good (complete an African Rhythms class) - 20 Uplay points
Stick To The Plan (complete an objective) - 30 Uplay points
Creature of Habit (burn at least 100 calories a week for 4 weeks) - 40 Uplay points
