I thought I'd create this solution as there's currently no comprehensive list of moves for the PC version. With that in mind, I have adapted Kovy88's excellent guide for the 360 version into this solution, the original can be found here: http://www.trueachievements.com/a155700/perfect-freeflo....htm
Planning is key to getting this right. Start off by making sure you have the following moves "unlocked" from playing through the campaign mode;

BASIC MOVES
1. STRIKE
2. COUNTER
3. EVADE
4. STUN
5. ULTRA STUN
6. AERIAL ATTACK

COMBOS
7. DISARM & DESTROY
8. BAT SWARM
9. MULTI-GROUND TAKEDOWN
10. COMBO BATARANG

FINSHERS
11. GROUND POUND
12. SPECIAL COMBO TAKEDOWN
13. BEAT DOWN

The best level is SURVIVAL OF THE FITTEST, as it has a good amount of enemies, with a very limited number of weapons/tough bad guys to deal with. I'd have a try doing this in ROUND 3, as there are just enough enemies. If you mess up - ROUND 4 has more than enough enemies to give it another shot.

In order to keep the combo running, keep double tapping space to jump over enemies, this can keep you in a combo infinitely, giving you breathing room to contemplate your next move. However, be careful not to jump more than once into an open area, you will break the combo.

PATTERN & CHECKLIST
Below is the pattern I am currently using, the aim is to do the moves that make you vulnerable to attack, towards the end of your combo;

EASY MOVES
1. STRIKE (Left click)
2. COUNTER (Right click) [When alert is over enemies head]
3. EVADE (Spacebar x2)
4. STUN (Middle click)

COMBO MOVES
5. DISARM & DESTROY (Y) [You can use this to get rid of the guy with the propane tank, giving you a moment to breathe. Just make sure you use the combo on an enemy with a weapon or it doesn't count]

6. BAT SWARM (R) [This will stun a large group, so make sure you do it in the center]

7. COMBO BATARANG (1 x2) [I've been told you should only do this when the combo counter is flashing yellow. If the counter is white, it won't count]

8. MULTI GROUND TAKEDOWN (T) [You should only have a couple enemies on the floor at this point, which is good, as we don't want to clear the area too much yet]

9. AERIAL ATTACK (Middle click, spacebar x2)

10. TAKEDOWN (E)

FINAL MOVES
At this point, make sure you have a small number of enemies left, as the following are likely to get you hit in the back with too much of a crowd.

11. ULTRA STUN (Middle click x3)

12. GROUND POUND (CTRL + Right click) [You can use this on the guy you just "ultra stunned" - just keep an eye out for enemies]

13. BEAT DOWN (Middle click, left click repeatedly) [I'd recommend using this against the last enemy]

This is a really tough achievement, my advice is find the method that works best for you and eventually you'll hit your peak.
