For starters, you will need to have played the game all the way through 3 to 4 times depending on your difficulty, however, my friend claims he did it in 2 playthroughs. The reason for this is because you will need to net (a cumulative total) 333 kills throughout any and all playthroughs, on one save file. Even if it takes you 14 playthroughs, you will eventually obtain the Heather Beam, but you should get it right around your third playthrough and it will unlock after the credits of the latest run where you've obtained more then required kills.

Now once you have acquired the Heather Beam, you will want to start an Extra New Game. I chose easy action difficulty and easy riddle difficulty just so I can run through it super fast. The only catch is that you will need to destroy 30 enemies or more using the Heather Beam, before reaching the entrance to Harry's apartment. For planning purposes you have the Mall, the Subway, the Sewers and the Construction site areas before reaching the apartment, where you will have plenty of kill opportunities in each area. My overall time was about one hour and ten minutes start to finish, and had about ten kills over the 30 mark. Just make sure you do not have any weapons equipped and you will be able to use the Heather Beam like any normal gun or weapon.

When you do your UFO playthrough, make sure prior to starting you have used the password to unlock the PrincessHeart costume, because it will increase the attack of the Heather Beam, thus killing enemies quicker!

All in all, fairly easy to obtain this achievement. It was my last achievement I needed and should only take you a 1/2 playthrough. Because it is a shorter playthrough, I know many people have also used this way to obtain:

Although I have not yet confirmed these myself, due to unlocking them on earlier playthroughs, but I have heard of people unlocking those two achievements. *Please confirm if it is correct and I will give you credit/add it to my solution*

"Not sure if anyone has told you yet, but YES, you can get the 2 saves or less and less than 500 damage ( and the less than 3 hours ) achievement by doing the UFO ending.

That is... Less than 2 saves, less than 500 damage, less than 3 hours, UFO ending. All of those achievements at once.

The not-so-average player, however, will get the 2 or less saves, 500 or less damage, and 3 hours or less on a regular playthrough with relative ease."
