This achievement is most easily unlocked during the tutorial. I will try to explain it so that new minecraft players won't have any trouble (note that I am using the default controls). It has come to my attention that the tutorial has been altered a lot but I have now updated this guide to accommodate that fact. For experienced players, there's a TLDR (Too Long - Didn't Read) near the bottom.

Getting a Fishing Rod:
Immediately after starting the tutorial, if you turn around you see a small lake with a chest in front of it about 5 blocks away from you. Inside the chest (left trigger to open) are 17 fishing rods. Press Y (quick move) on a fishing rod or manually drag it to your inventory. Exit the chest and equip the rod by pressing the left/right bumper until it's selected.

Catching a Fish:
Pressing left trigger with the rod equipped throws the line out, do this into the water behind the chest. Wait for the red/white bobber to go below the surface and then press the left trigger again to catch the fish which will be added to your inventory.

Making a furnace:
Go to the unfinished house in the middle of the area and destroy ten or so of the wooden blocks that the house is made of (hold right trigger until it breaks) and then pick it up. Now you need to make a crafting table, press X to open the crafting menu and press right on the d-pad 3 times to get to the crafting table. Press A to create it. Now that you have the crafting table, equip it and place it with your left trigger. Then open the crafting table (left trigger) and go right once and press A to craft some sticks. Now hit the right bumper once and press A to make a wooden pickaxe.

With your pickaxe equipped, head over to the stone walls surrounding you and chop off 8 stone blocks (hold RT with pickaxe equipped). Go back to your crafting table and in the same slot as the crafting table in the crafting interface you can press down to reach the furnace. Press A to create it and then place it in the same way as you did the crafting table.

Cooking the fish:
Open the furnace with the left trigger and put the "raw fish" into the ingredient slot and the "wooden planks" into the fuel slot. Wait for the fish to cook and then move it to your inventory to unlock the achievement.

TLDR: There are fishing rods in a chest next to a lake and planks (for fuel) at the beginning of the tutorial. Craft a furnace and cook the fish.

According to commenter CONKER1182 you don't actually need to catch the fish. You can cook one given to you by another player and still get the achievement. This has been confirmed by myself.
