This achievement is part of the story and cannot be missed. There are some tricky qualifiers I'll explain for the achievement-minded.

The pre-requisites for this quest are to travel to Gran Soren and then speak to Ser Maximilian. Max will bestow the Writ upon you and you will be offered 4 quests to go on:

• The Cypher
• A Fortress Besieged
• Seeking Salvation
• The Watergod’s Altar

If you solve one of these quests, Max will offer to take you to see the duke (which will unlock the achievement).

HOWEVER, THIS WILL END THE OTHER THREE QUESTS HE OFFERED AS WELL AS EVERY SINGLE CURRENT QUEST YOU'VE STARTED.

Instead, you should talk to Max after each Writ quest to turn them in and get experience, but ignore the dialogue option to speak to the Duke. Even if you accidentally choose that option, he asks you if you're sure you want to to speak to the Duke, lets you know this will close all your current quests, and gives you another opportunity to back out of the dialogue.

This will not close notice board quests, so if you've killed 14/45 rabbits, you don't have to worry. It will fail side quests you haven't completed and prevent you from starting certain side quests that will block you from getting the Hero achievement on this playthrough.

To test this, I copied my save onto a thumb drive and loaded it. I then told Max I'd like to meet the duke. I had 11 quests at that time, and after I told Max I definitely wanted to meet the duke, the game auto-saved and only one of the quests failed. The one which failed was a quest where I had to check multiple bandits' lairs for a book. I had looked at one lair but not the second one, which is why the game considered the quest "in progress" and failed it. The notice board quests did not fail or reset.

I then re-loaded the save of my hard drive, completed the book quest (it was called "A Troublesome Tome" IIRC) and spoke to Max. This time, nothing failed, and I continued with my playthrough.

The following is a list of which quests are failed or cancelled when you speak to the Duke. If you are going for "Hero," check your completed quests to make sure these are finished before telling Ser Maximilian that you would like to speak with the Duke:

EDIT: Allgorhythm notes that: Whereas this is a non-missable quest, be very careful when exiting the castle. As you exit, stay close to the main door. You will see a lady tending the gardens on the right side. She is Aelinore. Speak to her before doing anything else. If you do not do so, you will not have access to her quest line - Arousing Suspicion and Duchess in Distress.
