This is only possible in the postgame, after you've beaten the final boss.

Follow the main quest "A Warm Welcome" in Gran Soren. While this quest will prevent you from ever entering the Noble District or Castle again, the trade-off is that you can enter the Everfall. When you enter, you will be able to hold the right trigger to grab a ledge. I grabbed the first left under me (South) and landed near a door. Approaching the door showed me the label, "Chamber of Confusion."

Apparently, regardless of what ledge you land on, the first door is always the "Chamber of Confusion." When you enter this door, you will speak to some NPCs and loot two chests. Enter the door ahead to fight the Evil Eye.

My party was a ranger (myself), a fighter (my pawn), and two rented pawns who were a sorcerer and a mage. My pawn and I were level 46 and my rented pawns were level 60. I attacked the Evil Eye from the sides, staying out of the path of it's main eye. I prioritized killing the stalks on the ground and its head, as direct damage didn't affect it until the stalks were dead. Once the stalks are gone, it drops and you can attack its distended eye, and then pummel it some more while it is in a "ball" state. It then grows the stalks back and you repeat.

If your magic-users attack relentlessly, you can tear it apart when it's vulnerable. The entire fight took me 10 minutes, and the first 5 minutes of that were figuring out what to do.

EDIT: DivStorm notes that you should bring Panacea, Cockatrice Liquor, or Secret Softener with you to this fight. The Evil Eye can inflict Petrification on you or your party which wil result in an automatic death if not cured quickly by using the items listed.

Allgorhythm notes: the achievement can be earned fairly early on and before post-game by going to the Rotunda of Dread in Bitterblack Isle provided you are playing the Dark Arisen version of the game. The Evil Eye is a random spawn in the Rotunda of Dread.
