This is a story based achievement and can't be missed. However, if you want details, SPOILERS AHEAD:

You need to defeat Great Wolf Sif, who lies in the Darkroot Garden. You can buy the seal for the door to cut straight through the forest (though it costs 20k souls), or you can go around the forest by heading towards the Hydra in the Darkroot Basin.

Great Wolf Sif can be pretty easily defeated on your own with the right weapons and strategy. Sif's greatest weakness is fire, so if you have any Pyromancy spells, a Fire weapon, or Fire Bombs, they will come in handy. The trick is to stay under his stomach where he can't hit you with his massive sword. Remember to two-hand your weapon to do more damage!

Alternatively, you can always just summon people in to help. REMEMBER THOUGH, turning Human can be a challenge, especially in the Forest. It is a very common PvP zone, so be prepared to get invaded a LOT. If you really don't want to deal with getting invaded, summon whoever you need to, and immediately run for Sif. Ignore anyone invading because the moment you "Traverse the white light", they get kicked from your game. Fighting Sif with summoned people makes the fight incredibly easy. My strategy when summoning others is to keep them alive, because they can't heal themselves without humanity or the right miracles. I will stand back and use Pyromancy or Miracles on bosses, and let my co-op partner tank the boss with weapons. Keeping my teammates healed is my top priority because you're almost guaranteed a win that way.

EDIT: o0ArA0o has given one possible fix for the glitch that's apparently causing this achievement not to stick.
"Ok, the only way you can do it is to kill The Great Wolf Sif again in new game+.
Achievement will unlock again but this time it will register to your gamecard.
The easy way to just drop the ring and pick it up again will not work."
