Not sure why this is missable. I would say it is very hard to get to a point where you are no longer possible to get this.

Some basics:
-There are 2 ways to get control over a town: peaceful or by attack.
-Peaceful = takes long but gives you no trouble
-Attack = is quick but may lead to the other nations attacking you
-The game has 60 cities in total. Each of the 4 nations can be reduced to a minimum of 2. So you can potentially get 52 cities.

At first I tried to get all 5 / 10 / 30 cities peacefully. But this is extremely tedious.

A general layout that worked well for me:
-At the start of the game I focused on the cities that produce the basic building materials: wood AND adobe bricks. These towns are: Charleston, Campeche, Curacao and Georgetown.
-These cities I build up as much as I could and got them peacefully. It will be very helpful if you have a large stock of woods and especially bricks in these towns.
-On all the other cities I tried to at least get the commission to build a warehouse and have an advisor. This way you can use the city menu (checking tavern and missions, warehouse, free crew, buy from market to warehouse, ...) WITHOUT sailing to the city each time.
-Whenever there is someone in the tavern that sells goods or offers to buy goods: accept it. It always brings a profit. And the stuff from the warehouse sells itself.
-Same for the persons that give you treasure maps. I always accepted them. Use this map to identify the location: https://drive.google.com/file/d/0B6TiZOQuebrPVFlqVktrUHl2TUk...
-Other than that you might want to keep the other cities small until you have captured them.
-Then build up a strong attacking fleet. And always keep an eye out where pirate nests are. But let them be where they are until you need them.

As far as I know there are no perfect trade routes as the markets are dynamic.


Your approach for attacking cities:
-Try to be above 75% reputation with each nation. That is enough to get a letter of marque when they are at war.
-Wait until 2 nations are at war and get a letter of marque. I sided with the nation that had less towns on the map to keep the balance.
-Attack the towns with lowest population.
-You will only lose reputation with the 1 nation you are attacking. Try not to go below 25% reputation with this nation. Ot they will attack your fleets. You should be able to get 1 city, maybe 2.
-Before you go below 25% reputation attack the pirates to build up the reputation. Sometimes it is enough to only destroy all the ships at the pirate hideout. And let the hideout live so you can use it later again.
-At some point the 2 nations will end their war and the letter of marque expires. Time for you to repair your ship, build up reputation and wait for the next war.
-If you never go below 25% reputation with a nation you don't need to upgrade the defence of your cities. The other nations will not attack you.
-If you don't have pirates at hand check the cities for easy tasks to build up reputation (here it comes in handy if you have a warehouse & advisor in many cities so you can check the cities without traveling to them).
BONUS: I know it is boring but whenever possible use manual attack (esp on the tougher ships) and capture as much enemy ships as possible to boost your own fleet. This also helps with some of the other achievements and you don't have to grind them later.

This solution is medium speed but solid and relaxing. It took me 30 hours to get the 30 cities and the majority of the achievements. And the remaining achievements were easy to grind.
