I don't believe this achievement is as easy as it's made out to be. Counters in Resident Evil 6 require pinpoint timing, and for me a little luck. You can't exactly spam RT and you have to be in range of your enemy so they'll attack. There are a few things to note beforehand however.

*This is easiest to get against zombies (as apposed to the J'avo), therefore Leon's campaign is your best bet.
*The hardest part is timing the counter right. Learning how zombies move when they're about to attack helps, because in the end it's all about timing.
*You cannot kill any zombies in between counter kills. Reloading will also reset your counter count.
*If you fail a counter, you're fine.
*If you get grabbed after failing a counter, and you're prompted with a timed even, hit it early or miss it on purpose. If you hit it on time, you will kill your target, but it will not count a counter, thus resetting your count.
*Zombies without weapons can be countered.

There are a few places where you can (somewhat) easily grab this achievement.

LEON (CHAPTER 2) - The Graveyard or The Catacombs
====================================================
The most popular spot, as far as I see, for nabbing this is the Graveyard leading up to the Cathedral at the beginning of Chapter 2 in Leon's campaign. In this area, zombies will spawn randomly.

If you're going for it here, look for enemies with shovels, as they're the easiest to counter. Stand in front of them and when they go to swing at you, simply time the counter correctly. There will be a cinematic if you did it successfully.

The zombies that don't have weapons can still be countered, but it's not as easy. When they stand in place and twitch, as they're about to charge you, wait until they charge and leap at you. You'll be prompted to counter just as they're leaping.

Just beware if attempting this in the graveyard: you can't counter the dogs, so avoid them at all cost. Also, Helena may gun down some potential counter victims. I haven't found any way around this.

*If you play through Chapter 2, you'll eventually reach the catacombs of the Cathedral, where you're pursuing (a boss). The zombies down here are similar to the ones in the graveyard, and they're more likely to have weapons like swords and torches. It may be a good idea to play through the level to this part, especially since, if you're playing as Leon, Helena won't be much of a problem with stealing your kills.

The Mercenaries
====================================================
The achievement is obtainable in the Mercenaries mode, and it's a lot easier. The Mercenaries presents you with a bunch of zombies from the start, and enough time to hunt them down. Additionally, the zombies always spawn in the same spot, with the same weapons, so it's not hard to find and remember where the zombies with weapons spawn.

I personally recommend the Mining the Depths level. Three enemies are available right at the start, two of which have weapons. Just beware, there is a Shrieker on the level (if he screams, zombies will go nuts), so try to avoid him. Also, if you take too long, dogs will begin to spawn, which could be a problem.

Grabbing this in The Mercenaries is a lot quicker and easier then doing so in campaign. The achievement isn't that difficult, luckily it's only three counters. Just perfect your timing and be careful not to kill any zombies in between and you'll be good to go. :)
