This is the 5th Level in the game and involves you following Death around trying to free 3 spirits. You will reach a central hub midway through this level and there will be 3 statues at one end:
See no evil
Speak no evil
Hear no evil
Pick one of these up and a portal will open. Complete the mini challenge through the portal (this may be fighting enemies, playing a carnival shoot out or proving yourself to the Amazing Deadpoolio). Once you have completed a challenge, take the statue back to where you found it and pick another up. Complete this for all 3 statues and once the 3rd one has been placed back on its stand, the achievement should pop.
