How to unlock the Ever Vigilant achievement

OK these are the 10 i found.

1. Level Facility: At the end just before you rescue Pavel before you climb the last ladder.
2. Level Sundown: Weapon cache in the white truck.
3. Level Sundown: Weapon cache by the over turned car.
4. Level Sundown: Weapon cache by the narrow building near the attacking vines.
5. Level Nightfall: In room with night vision.
6. Level Nightfall: Room adjacent to the night vision room.
7. Level Nightfall: Upstairs in the building with the escalator attached to escalator opposite side of bathrooms.
8. Level Nightfall: Upstairs in the building with the escalator on opposite side of bathrooms attached to far end of escalator.
9. Level Nightfall: Upstairs in the building with the escalator on opposite side of bathrooms small room containing weapon cache.
10. Level Contagion: On your way to rescue Anna you will come to an area that is an abandoned railroad looking area colored a bit green. You will duck into a hole to get to this area. This area is near the beginning after you leave the room where the guard is doing inventory.

11. CONTAGION - after crawling through the vents and witnessing Anna being held hostage and two inept executioners, you will come out onto a train track which leads to a train with a Heavy loading boxes onto it. Go into the room he is loading from (there are two enemies here as well), and through the door on the opposite end. To your right will be the trap.
12. DEPOT - at beginning when you emerge from the tunnel, bear left passed the guardhouse and pass the garages on your left. At the end of this row of garages will be ruins of one; enter here and turn left to find the trap and an Enlightenment point.
13. DEAD CITY - beginning of the level, take the first right through a doorway into a building, will be on the right.
14. DEAD CITY - after the playground flashback, you will drop into a hole in the ground; hug the left wall and the trap is in a doorway at the top of a short flight of stairs.

Thanks again Baloo Darling for your contribution. This is good for people that may have missed one since this has to be done in one play through.

In the below solution Glenn 46 is correct. This does have to be done in one play through as does the musician achievement.

There is a 3rd trap in The Dead City. You jump out a window on top of a truck and it's in the ruins to the left up a staircase. It's before you climb the ladder to the roof top.

I left this message because that was the last one I needed and I had missed too many throughout the game.
Achievement unlocked! Woohoo!

I tried doing this using chapter select and it didn't work. I know I've disarmed 10 traps, but the achievement didn't pop. I'm going to be doing a full playthrough soon to get the good ending, play all of the musical instruments and disarm all of the traps. Hopefully it unlocks then.

There's also a trap in the level The Garden:
Near the beginning of the level, there is a path leading towards a hole in the right-hand wall, into a flooded store with can alarms and a safe. Entering this area earns a point.
At the same split where you can reach the flooded store, you can also turn left, crossing a log and a flooded pipe. Right after the pipe will be a tripwire; disarm it or avoid it, then crouch ahead and you will receive a moral point.

Just going to add this. YOU have to do ALL 10 in 1 Play-through. I had about 5 or 6 in my first time through, then I was going doing Ranger mode and scrounging everywhere to find ammo and found a lot more traps the 2nd time through but did not get this achievement till I was well past the 4 or 5 I thought I needed, as I had gotten the 5 or 6 traps I got the first time during this play through with no achievement popping. So I think this is in one play through not cumulative but if I am wrong please let me know and I will drop this post.

I noticed that there were som errors in the 1st guide so im putting this one up aswell. Theres only 1 trap in contagion and 3 in dead city. In total there are 14 of them. And as ben said on the other solutions they have to be in 1 playthrough. All credit goes to wikia

EDIT. There is 1 minor error in this guide. The level contagion is BEFORE .Depot. So u dont miss that
