This is my first guide, I'll try to explain this as best as I can.

This achievement is obtainable on chapter 4 while trying to find Rachael in order to obtain the lift key. I did it on the normal difficultly. I had eight grenades and three B.O.W. Decoys. The two weapons I used were the MP5 (+ 10% damage, stopping power +100%) and the M40A1 (damage + 20%, firing rate + 30%, critical +30%, load capacity 20%). One important note: when you hear Rachael say "it hurts," she will focus on running away from you. The more injured she is, the slower she will move.

Make your way into the med lab area where you first came across Rachael. There will be a auto save when you enter the room (you can load this check point if you mess up). Near the window, you'll find her journal. After you read through, a cut scene will play, revealing Rachael. With the MP5 and B.O.W. Decoys equipped, make your way up the stairs to the landing in between. From here, I turned and fired 30 rounds at Rachael (She ran away if I fired any additional rounds). She should be just about next to you by this point. Throw a B.O.W. Decoy on the landing and make your way up to the top of the steps as she runs towards the explosive; she should be knocked to the floor by it. Once she's laying down, fire a quick burst with the MP5 or 1 to 2 shots with the rifle, then throw 1 to 2 explosives (either type) at her as she begins to stand. The explosions should knock her back down, allowing you to repeat the cycle.

Should she get away, She'll appear in two other areas before the cafeteria: the long bathroom area (from a vent behind you) and in the following hallway (before the second door leading to the stairs).

This took me several tries, but I finally got it. Don't forget to scan her for the Research Complete achievement.

Props to Lord of Tarkana for the guide that gave me the technique to do this, my method is essentially just a slight modification of theirs.

The first thing to note is that my first run through was on casual difficulty, so keep that in mind when I am describing how many shots I pumped into Rachael.

The second thing to note is that I unlocked the PSG1 partway through this run, so I had it to work with going in to the fight. It is critical to the method I propose as the trick to it is to bring two sniper rifles since you don't have a magnum yet for big damage output.

My loadout was as follows:

1) M40A1, with Damage 1 and Firing Rate 2.

2) PSG1, with Damage 2, Long Magazine 2, and Crit 1. This gives you 840 damage and 7 rounds/mag.

3) MP5 with Bind 1 (slap whatever else will fit on it, I basically forgot to upgrade this one as I was focusing on the rifles)

I had 9 grenades and 8 decoys. The SMG will have 50 rounds to work with, the PSG1 will have 7, and the M40A1 will have 8.

After the cutscene make sure you have decoys and the M40 equipped, then make a beeline for the part of the landing that allows you to see where Rachael comes out of the vent at the top of the stairs. Try to get a potshot in on her before she starts coming towards you, and once you fire that shot, start reloading as you run down the stairs. Back up the left side hallway that leads to the examination room door (if done correctly you should be facing the dead end of the hallway that would be a right hand turn as you come down the stairs, it has a couple lockers at the end). You should be done reloading as you back up just shy of the corner that leads up the stairs, so throw a decoy at those lockers. As Rachael comes down the stairs and heads for the decoy, try to get another shot in on her with the M40. The decoy should knock her down, now equip grenades and try to get a headshot with the M40 while she's grounded. Throw two more grenades at her as she's getting up and try to get another headshot with the M40 when she falls. Throw two more grenades as she's getting up and equip the PSG1. The grenades should knock her down again so use that opportunity to unload as much of the PSG1 magazine into her head (if possible) or body if not, but make sure to throw two more grenades as she gets up again. Finish emptying the magazine into her, then pull out the SMG and start unloading OR throw more grenades if geography permits and THEN unload once she falls (if she's halfway up the stairs, as she was for me at this point due to her taking some unfortunate fall directions from prior grenades, grenades may not work that well, hence me just unloading and hoping for the best).

With any luck she she will stagger and/or fall down with the SMG assault which allowed me to equip the M40 finish her with a shot, though results may vary on whether the stagger/fall occurs from the SMG fire and how many M40 shots will ultimately be required to finish the job. Prioritize just hitting with the M40 as she should be very damaged by this point.

At the end I had completely emptied the MP5 magazine, completely emptied the PSG1 magazine, had a half empty M40 magazine, and 3 grenades left.
