Just got my 200 in this after years of having it in my arcade. Spent about 4 hours alone working on this achievement. And it's by far the hardest achievement in the game. Lose no humans in the first 3 waves. Sounds simple enough, but the key to getting this is having a combination of skill and luck. You must be patient and persistent if you want to obtain this one.

First off, play wave one without losing a life and without using a bomb. And of course without losing a human. If you lose a life here, just start over. You'll see why. This stage only has landers on it (the default green enemies.)

Second wave becomes increasingly difficult. The landers are slightly faster now, with the addition of one Pod (once you shoot it, a bunch of little ufos named swarmers come out.) And there will be three bombers (pink enemies on the hud above your game.. they spit out stationary bombs to try and kill your craft by running into them.) While I mention the hud, always pay attention to it. When I play, I spend half of my time looking at the hud. Seeing where the enemies are, going in that direction and then looking at the screen to kill them. And then back to viewing the hud again.

Obviously the most important part of this achievement is making sure the landers do not scoop up any humans. So they are first priority kills. However taking out the 3 bombers with a smart bomb early is just a good idea to make sure they don't lay any bombs that continually get in your way. Try to save the pod kill for last, if you shoot it early, the swarmers might mess you up if you don't dispatch them with a bomb quickly. (I have found that sometimes bombing the pod will also destroy the swarmers before you even see them, and sometimes it doesn't.. a matter of luck I guess.)

Now here is the key to this wave and the third wave. If you hear that sound that lets you know a human is being taken and you look at your hud and see a lander has just grabbed up a human and is heading to the top of the screen, judging if you are able to make the save or not is everything. If you know that your just not going to get over there in time. Intentionally kill yourself as fast as you can by running into the closest enemy you can find! Yes you lose a life, but you also save that human that almost died. And that pretty much is the strategy in the video link below.

The 3rd wave is much like the second but of course it's harder. Now the landers are much faster. And there will be 2 or 3 pods and another set of bombers to deal with. Do your best to take out the landers ASAP and deal with the annoying other enemies soon after with a bomb. Try to get the bombers when you see them and try to save the pods for last.

This is a hard achievement. And most will give up. Study the video courtesy of p3rvypri3st on youtube. And be persistent.
