This achievement is awarded for fully healing the Angel of Valhalla, a white Chocobo which you must save in the Wildlands region of Nova Chrysalia. While there are many ways to restore the Chocobo to full health, and therefore unlock the achievement, this solution will focus on the most ideal path in which the fewest items will be wasted which could be used for other purposes.

3-1: The Angel of Valhalla - When you originally arrive in the Wildlands, a scene will direct you to Canopus farms, which lies in the center of the grasslands. You can direct yourself towards the farm by looking for a windmill to the east.

Upon arriving at the farm, speak with Dr. Gysahl, and subsequently all NPC's with marked with a speech icon. This will create a quest marker to the north-east. Leave the farm using the north east path, and speak with the hunters found there. Now you know your exact location: the City of Ruins.

When you reach the quest marker, you have to take on a very difficult Chocobo Eater, but fortunately it has been weakened quite a bit. You will want to have Passion Rouge with Thundara, Electronica, and a third schemata such as Nocturne with Sparkstrike if you have it. Try to perfect block when you can, and have some healing items just in case. This battle shouldn't cause to much trouble, no matter how early in the game you are attempting it.

3-2: Healing an Angel - Now that you have rescued the Angel of Valhalla, you can go about healing it. There are many items you can use to heal the Chocobo to full health, but the basics are that you want to reach 400 points of healing in order to unlock the achievement. Talk to the Hunter Chief just outside the village to receive a Chocobull, Serala will give you Gysahl Greens, and the Inkeeper will give you Fuzzy Sheep Milk. Give all of these to the Chocobo. A full list of items you can use to heal the Chocobo are as follows:

This chart is best viewed at a resolution of 1280x720 or higher. If you can't view all of the last column, don't worry, the important parts are covered later in the solution.

Having used the items mentioned above, you now have 95/400 required points, but a little more is needed before you can ride the Chocobo. Head to the south-east of Canopus Farms, and just near the small path heading into the woods, you will find Hunter Hopeful, who will give the quest "The Right Stuff". If you don't have 6x Vibrant Ooze, simply fight Miniflan in the area until you do. The Hunter Hopeful will reward you with a Mystical Meal, which you should bring back to your Chocobo and feed him with.

This brings The Angel of Valhalla to 145/400, and more importantly, to riding strength. Hop on and head towards Jagd Village, a settlement in the middle of the Jagd Woods to the West (Follow the path the Hunter Hopeful was near).

If you are doing this before 18:00, you can collect Chocoborels along the path (small mushrooms), and kill Raflessia for Slug Sweets (available any time other than 16:00-18:00 when Anubys' replace all other species). When you reach Jagd Village, you can have Cardesia make a Chocobull if you have 8x Chocoborel and 6x Slug Sweets. Feed this to your chocobo to bring the total to 185/400.

Go to the west within the village to find Nadia by the Chocobo Pens. This will unlock the "Chocobo Cheer" quest, thus allowing you to use the incredibly useful Yeul's Flower for healing. There are four flowers in the Wildlands, each near the four extremeties of the map. The one in the west is the only one which is behind a timed gate (7:00-19:00), while the one in the north can only be accessesed once you Chocobo can glide (250 points).

If you are doing this in the morning as recommended, head west first and get that Yeul's Flower first, bringing you to 235/400. Now head to the City of Ruins (where you fought the Chocobo Eater) and get the Yuel's Flower there, bringing you to 285/400, which unlocks gliding for your Chocobo.

NOTE: At this point, you can continue on with the Wildlands quest line, and even complete it. You do not have to fully heal the Chocobo in order to complete the storyline of the region. Thanks to Celluloidbuff for helping me to clarify this.

Head to the south now to Aryas Village and grab the flower down there (335/400). Go to the sheep pen in Aryas to complete the "Fuzzy Search" quest, and feed the Angel the Fuzzy Sheep Milk reward (365/400).

The final 35 can be obtained several different ways, but grabbing the northern Yeul's Flower is recommended, as you will not waste any other items. Head through Jagd Village and glide across the northern gap. Depending on your strength, you may want to avoid Protoflorians in this area. At the first large open area, take the right-hand northern path. At the T, head west, and follow the path north. When you reach another T, head north-east. When you reach the large open area, you will be up on a plateau. You must jump across a few of these in order to reach the flower. If you fall, watch out for Chocobo Eaters as you find your way back up, as they are likely far too powerful at this stage. You are able to reach this area through an easier path (see map), but you might as well traverse the woods and look for any collectibles and ingredients on your path.

Eating this flower will bring you to 415/400, thus fully healing your Chocobo, and earning you the achievement.

EDIT: Celluloidbuff contributed an alternate way of finding any/all of the healing items needed for earning the achievement:

I just wanted to add that the achievement will still pop if you continue to give your mostly healed gliding Chocobo any of those ingredients above, even when you've completed 3-2 of the Main Quest in the Wildlands. It doesn't have to be completed while 3-2 is active.

The easiest to obtain by far are Gysahl greens, which only take 6 game-hours to grow, and there are four plots in Canopus Farms to harvest from. One harvest yields 100 points. Simply plant four new seeds, complete others quests, and harvest again.
And my response to this:

Yes this is completely true. I don't think I said that you have to complete it during the quest line, and I do mention that you can finish the healing any way you would like, but the clarification is nice, so thank you.

The reason why my method involves travelling the entire wildlands, is because you have to anyways in order to complete side quests, main quests, and canvas of prayers quests, all of which contribute t other achievements. I will add your comments to the solution, however, thanks kindly.
