As someone who got immensely frustrated with this achievement, let me pass on some advice (a lot of credit goes to Beavis XBL for this as well) when it comes to the fights:

If you find that you are having a lot of trouble getting the fighting medals, keep things simple: keeping your combination up as high as it can get is the key to getting 3 medals.

I was trying to do fancy Shuriken Attacks (X + A) and Kabuki takedowns (LT + X then Y), which in the other achievement "Bruce's Revenge" I found quite easy to get 3 medals, but in the Extreme fights for this achievement I found that my combos would break too easily; the Kabuki takedown too often broke my combo. Shuriken Attacks will not break your combo but ensure you can get the button execution done right so you don't break your combo.

Also ensure to load the bad handicaps (reduced health, no special combos, no gadgets) in the first challenge. It will make the second and third challenges in the set of 3 a lot easier and there's enough enemies in the first challenge to get 3 medals even when really handicapped. Always use Reduced Health in the Fighting rather than in Predator, as in Predator you get just one bar whereas in Fighting you get about half your usual life but it will refill at the end of each round.

Wise advise from Beavis XBL:

"forget gadgets, fancy takedowns. just leap over the guys with A+A and strike with X, leap, strike, etc. when you get to a combo of around 20, start listening for the knockout sound. it means you have knocked a guy out of the fight. i wait for that sound because up until that point i can get some punches in, building my combo. at this point, you want to start doing takedowns, so just Y+B. keep your combo going and every time you get the chance to do a takedown, do one from this point on. reason being that takedowns give you 50x your combo. you dont want to do it too early because you want the highest possible combo for the multiplier. beatdowns give the same amount, but you risk being punched if you aren't fast enough. so unless you have a guy all by himself, skip the beatdown. the only other benefit of the beatdown is that you get some more punches in, so it would raise your combo. not worth it in a crowd."

You don't need to be too fancy here except to evade (LT + A A) when you get too surrounded or are facing an enemy you cannot strike directly (e.g. those holding car doors or electrified prods). Also learn to time your counters when being surrounded by simple thugs. If you can keep your combo up you can often surpass the 3 medal requirement very easily: in one situation getting 3 medals required 72000 points, and I just wasn't able to nail this down. Eventually with enough practice, I kept my combos up very high and ended up getting 125000 points for that fight. Simple once you know how!

Just keep practicing. Your muscle memory will get better. This achievement took me about 3 weeks to get but in the end I got the last 3 lots of 3 challenges (9 medals) in two sittings. Don't be discouraged; you can do it!
