You need to experience 5 slow-mo scenarios for this achievement to unlock.
To be honest, I missed it in level 1, but there is a good section in level 2 when you are being chased by the monster (2-3).
At the end if the level the monster will push a tree at you, and you have to create a branch to stop it landing on you.
All you have to do is draw the branch to pass the slow mo section, then just kill yourself, and repeat another few times.
The restart is exactly where the tree falls, so it will take only 5 mins to replay this section to get the acheivement.
