To get this achievement, you need only to 100% sync the Story Missions. You do NOT need to do any of the side missions, and do not need 100% sync on any that you do.

I believe (though I have not tested) that you DO need to go back and do the Sequence 8 Mission "Connor's Way" which is not unlocked until after you beat the game (and has no 100% sync requirements other than finishing the mission).EDIT: Commenters have pointed out that this mission does not appear to be necessary.

Those who have played other Assassin's Creed games may be sweating this one, but almost all of the 100% sync requirements are TRIVIALLY easy. Most of them I got on my first try with no effort. Here's a couple that gave me trouble, and how I beat them:

7-2: Supplying the Revolution
This one has you escorting a raft, and requires you to not swim, keep the raft above 50% health, and use only melee weapons. The important thing to make this easy, is that you do NOT need to do all three in one playthrough. Do one time where you stay on the raft and use your pistol , and another where you are free to swim and use melee. One of these you are sure to keep the raft healthy. Also, if you are having trouble with raft health, know your checkpoints. Any time you reload last checkpoint, your raft starts with 100% health, so as long as you haven't seen the red text notifying you that your raft went below 50%, you can reload and get a fresh raft.

9-2: Confronting Agate
This one you need to take no damage (you also need to complete the mission in under 5 minutes, which is trivial). This means you need to kill 15 people without getting hit once. First Agate will attack you before the halucination sets in - sometimes he can attack right as the halucination triggers, making it impossible to counter, so instead, i just turn and run around the pen so he doesn't get the chance to. Next you will have 5 groups of 3 enemies. Most of them you can counter kill (B => X) or at least counter disarm (B => A => mash X). However since there is zero margin for error and I had a lot of trouble remember which enemies had to be taken care of which way, and there were shenanegans where an enemy would hit me before they had fully phased into the world, I quickly got frustrated here. The trick is using SMOKE BOMBS. If you drop a smoke bomb just before they fully come into the world, they will all spawn choking and can be killed in one attack, even the specials.

9-4: Erudito
Again, the goal is to take no damage. For this one, you start off facing about six baddies at once, and your mark will not enter the fray until you've killed some of them. Make sure you have the pistol equiped along with any melee weapon (i prefer sword). Carefully counter some and study which you need to counter kill and which you need to counter disarm. As soon as your mark joins the fight, hit LB to activate chain kill and make sure she is one of the targets (this is why you need the pistol equipped). Many people have pointed out in the comments that smoke bombs work great on this mission as well.

NOTE ABOUT GLITCH: If you, like me, complete the final mission you need and don't get the unlock, go back into the DNA and re-complete any 100% sync. (1-6 is probably easiest). The achievement should unlock then.
