You can easily get this in the very first mission where you get the berserk darts.
If you're already past it load up Sequence 3 - Vanishing Slaves. Make sure that you also have at least one smoke bomb left when you're going into the mission.
Now when you get to the enemies you want to use the berserk poison (use RB and the right stick to equip it). Move near the enemies and make sure you hit the soldier with the white clothes with your dart! After you hit him move into the enemies and use a smoke bomb on the enemies to the left of the guy in white. This will stun them so the Berserk can go crazy on the remaining soldiers.
