First, buy the England EXTRA Story of Qualifying in the EA catalogue. It allows you to get an easy challenge to get this achievement. Now load up the challenge where you must score 5 goals against San Marino within the last 13 minutes.

When you load this up, Cleverley has already scored 3 goals and Shelvey has scored 2 goals. While doing the challenge I scored 2 more goals with Cleverley and the achievement unlocked. You do not need to finish the challenge for the achievement, only score the needed goals.

EDIT: neeker75 has told me that it is not always Cleverley who scores 3 goals, but the 5 goals will always be spread out around the team no matter what.
EDIT 2: DigestedGolem says you can check "Match Events" on the menu once you have started the challenge to see who has scored the goals.

Note: You can do this however you want, but this solution means you only need to score 2 instead of 5. Also, if you down vote let me know why.
