    There are a total of 16 QR Codes that need to be scanned within the game before you are given the final investigation and can complete this achievement.

    To have the locations appear on your map, you will need to progress through the story and unlock the ctOS Towers, but you can scan them early if you follow the guide.

    TheAnonymityMy tracker says I have 15 and nothing else shows up on the map. Finished the game. Haven't done all cTOS breaches but I have every tower unlocked.

    Fukuda97Got mine today. this one glitched to me and i had to delete my savegame from xbox and cloud server to play a 2nd playthrough again till begining of chapter 3 to start scan the QR codes to unlock the achievement.

    There are 16 QR Codes to find in the QR Code Investigation, the locations can be revealed on the map after you unlock every ctOS tower. You then need to go to these locations and scan the Codes. The codes may be split apart on different buildings and only line up to be scanned when viewed from certain cameras.

    Once you have scanned all of the QR Codes you will unlock the final mission and then need to complete the final mission to catch the hacker and unlock the achievement. ( You might need to have progressed into Act 3 of the campaign to unlock the final mission)

    King fiShiZits not, if you would read my guide, you would know that you unlock it at act 3 ;)

    VirusTk GaMBitHi, unfortunately I did not get the achievement. My progression bar shows that I have completed every code. And I got the achievement for all the investigations as well. I think it just glitched on me. Is there anything known how to solve this. I really don't want to play through the complete game again

    VirusTk GaMBitHi, I just wanted to add that you need to finish the 10th mission of act 2 to get the final mission to become available. As I said earlier, the achievement glitched for me and I had to play trough the game again. When I finished the 10th mission in act 2 I got a phone call from this hacker group that made the final mission available.

    Videoguide to the Read-only Achievement

    The codes can be found on buildings and other structures.
    To align the code you need to find the right camera.
    Follow the videoguide and you will be done in no time at all

    There is 17 QR codes. The last is in an intrusion kind of mission
