This achievement will unlock once you reach level 70 in hardcore mode. In hardcore mode, you only have one life. Death is permanent. You do not need to complete story mode to reach level 70, though I would recommend it for adventure mode (and also to get kill boss achievements from hardcore mode). Your stash, gear, gold, etc. does not carry over from normal mode. Be careful if you join someones game, apprentice mode is not available in hardcore so your stats will not scale.

Adventure mode offers 5 bounties per zone. Each bounty gives gold and experience on completion. The most efficient bounties for experience are the clear _____ bounties (usually max 1 per act). In particular, "Clear the Hell Rift" in Act IV seems to be by far the most efficient way to level. Above level 60, it gives over 2 million experience per completion and takes less than 10 minutes (repeatable by backing out to menu and selecting mode again). After completing story mode, I would recommend completing this bounty over and over again. It can get boring, but isn't that difficult to do quickly (and safely).

Some other tips:
-Play on normal. Harder modes give more experience and rewards, but it is not worth the risk.
-Play the class you are most comfortable with. Knowing how to live in difficult situations is important!
-Take it slow. Rushing or allowing enemies to swarm you isn't worth saving a bit of time.
-Stack vitality. Having a huge HP pool helps. Don't leave attack/armor low, but when making gear decisions (such as gemming), vitality is your best friend.
-Use elective mode and use your defensive skills . I had 2-3 equipped at all times to support survivability.
-Keep your gear upgraded. If you haven't upgraded a piece from drops in ~10 levels, I would recommend crafting something. With the enchanter, you can reroll stats on gear for more vitality, life on hit, or other useful stats.
-The templar is a useful follower. Heal keeps you alive and the stun is good for crowd control. It can stun bosses, too.
-Complete events. Events give good experience bonuses and can give good loot.
-Grab pools of reflection (yellow wells) for bonus experience. These last through logouts.

I used the above guide on my way to level 60 and found it helpful. Note. I never played in a party (always solo) and still did just fine. I didn't want to risk a laggy connection leading to death.

It is probably possible for higher level friends to boost you by completing quests while you sit in town, but I have no experience in hardcore coop so I am not sure. Feel free to let me know and I will update the guide.

edit via Senyth: When playing solo, you can pause the game and quit to get yourself out of a dangerous situation. This will safely return you back to town.
