  This is a pretty basic one. Go into duel mode, select survival, pick your team and cards and begin.
  
  The achievement will pop once you lose and get to the results screen.

  I've done this, but the achievement didn't pop for me. Have I ended up with a glitched save and require a fresh start to the game (i.e. wipe the save file and start over) ?

  Popped even before starting the mode, just on the loading screen when you can select cards and music.

  Yeah it popped at the start, I didn't even need to play anything
