It appears as though each episode has a separate set of fortunes -- or, at least, is more likely to give certain fortunes over others. Fortune cookies spawn on the coffee table in David's apartment; you can only read three at a time, and each read is randomly selected from the list for the episode. You'll need to reset the fortune cookies in each episode separately to obtain the achievement. Each fortune is worth roughly 3 percent of the full achievement, assuming each is equally weighted.

For the prologue, you will not be able to dive in and out of David's apartment, so you will likely need at least two play-throughs or replays to get all of the fortunes. After gaining control of David, read the three fortunes on the coffee table, and then save at the phone. If you are not replaying, you can save at the phone before reading the fortunes, quit to title, and continue should you not get what you want. For episodes 1 and 2, you can replay them from the title screen, or get the fortunes while playing through the story; while within the episodes, don't exit to the title screen, as replay does not save story progress! Instead, simply reenter the dive and then re-exit the dive to reset the fortune cookies. Episode 1 starts within a dive; you can exit it after a brief cutscene. For episode 2, you will need to play through the story until after the stunt sequence with Amanda, at which point you can farm for fortunes.

As a helpful tip to speed this process up when using the controller, you can double-tap cn_B to skip cutscenes, single-tap cn_B to center the hand cursor over the back button, press cn_back pretty much anywhere to go the the game menu; from the game menu, you can quickly enter/exit the dive by tapping cn_upcn_A.

The following is a fairly complete list of all the fortunes I found while attempting to finish this off.

Prologue (5):
