To be able to get this achievement you first need the following evidence to reach the right conclusion at the end of the case:

    - Stolen valuable papers
    - Looking for papers
    - Neligan's innocence
    - Harmless flirting
    - Unexplained presence
    - Hurtley's innocence
    - Feat of strength
    - Pouch proves guilt

With all this information you'll be able to reach the conclusion on the Deduction screen that Cairns was the killer. When asked to "Condemn" or "Absolve" him, you must select the "Condemn" option, which is A, to be able to fight him.

Cairns arrives at Baker Street expecting to talk to Captain Ahab, ad when he realizes that he has been set up, will punch Lestrade and initiates a fight with Sherlock. To beat him, you have to aim within the circle around his face with the left stick and quickly press "A" when the circle turns blue. After three consecutive successful hits you have to press "Y" in a QTE to knock him down and beat him for good.

If you fail simply press "X" after the conversation with Lestrade to make use of the "Replay Ending" option and repeat the above steps.

GuitarHeroAeroBugged for me too. Tried replaying the ending several times, tried starting from a new game, nothing. Oddly enough this is the only achievement that hasn't popped; even the pseudo-platinum popped for me - so I'm assuming somewhere it registered that I've done this, but hasn't given me this achievement.

NasalEquation11I have attempted this achievement a couple times and have yet to see it register.
UPDATE: I deleted from local and then from everywhere. Replayed this case and the achievement popped.
