  This is missable because it isn't really story related and you can carry on with the story without ever doing it.
  
  Once Chapter 4 starts you need to enter the Doctors brothers house and kill him.
  
  He isn't hard to take down and I managed it with a Flash Bolt and 2 Shotgun Shells to the head.
  Once he's dead you'll experience the vision and net the Achievement.

  I missed this on my first run as I thought that building was the one I was supposed to go in to progress the story, so I instead followed Leslie. Of course, once you do that, you get trapped down there and have to deal with a lot of stuff that leads to Chapter 5. No going back.
  
  So, once the Chapter ended and the realisation sunk in, I had to reload a save right before I followed Leslie and deal with the Doctor (A shotgun blast to the head followed by burning his body is all you need).
  
  I had actually forgotten about this achievement, so it was a surprise when I got it. I just was annoyed I didn’t get the chance to explore the house and wanted to come back.
  
  I then had to repeat that whole Leslie section again.

  Literally was shitting myself when it didn't unlock cause I skipped the cutscenes, haha. Thanks for that heads up, you guys. 👍

  At the beginning of Chapter 4, enter the first house on your left. Once inside follow the hallway on the bottom floor until you enter a small surgurical room which will begin a short cinematic.
  
  After the cinematic, take out your shotgun and kill the doctor, which will trigger another cinematic and then unlock the achievement/trophy.

  In Chapter 4 you will want to enter the very first house on the left. Go to the first floor and when you get to the area a cut scene will occur. After wards kill the enemy and yet another cut scene will pop up. The achievement unlocks after the second cut scene.
