The new door half-way up the stairs in the main Hall leads to a cemetary like area with a long stairway leading down to a crypt.

In there hangs a coffin which is held by 4 chains. For each chain you have to find a Death Mask... once all are placed the coffin will fall down and you have to fight this Crimson Head Prototype which didn't seem any different to me than other crimson heads. Just bring your Shotgun and you should be fine. You can easily run around him and avoid a running attack -> after that you can get a free shotgun hit.
