To get a high score, you need to:

1) level up (level 7 or higher should be sufficient, depending on your skill) - each level will raise your score multiplier.
2) play chords correctly as soon as they appear on the screen. Every correct chord builds up your combo, giving you higher scores. At the beginning you'll just get a 1K point or so per shot, with higher combos you'll get a few hundred thousand per shot. So DON'T LOSE YOUR COMBO! (You lose your combo if you don't play the chord fast enough or if you play the wrong chord).

It was actually a matter of luck in my case.

First of all, there seems to be a problem with my setup or something, because the game doesn't register some of my chords, even if I play them perfectly..... I lost a few combos because of C#m, the most problematic chord for me. I'm a decent guitar player, I know my chords, no problem there. But the game seems to do what it wants to do.

The good thing is that the chord sequences are kind of random, so sooner or later you'll get a sequence with chords that the game 'reads' well. For me it's a combination of Em, Am, G, C, D.... So just play along and you'll get good chords.

Sometimes when the game didn't register my chord, I tried playing the chord again immediately, strumming slower. It usually worked (with the exception of C#m).

Just play through the game, levelling up. The score multiplier is important. I started getting scores of 7-9 million when I got to level 6 or 7. I was so close.

If you're lucky, sooner or later the game might throw at you a sequence of identical chords. I got a sequence of about ten D chords in a row, resulting in a few dozen million score!!!!! PPop, achievement unlocked.

BOTTOM LINE IS - don't give up, play along, you'll get lucky, the achievement will pop.

Edit: Senseless Ogre (in his X360 solution) made a list of the most common chords you will need for this minigame: Em, Am, Gm, Dm, C, F, G, D, F5, G5, Bb5, B5, C5, D5

And I have to add C#m, obviously.
