You just need to complete all 6 missions within Operation C to unlock this achievement.
Occasionally, after completing a mission, the game will shut off, which causes any completion achievement to not unlock at the appropriate time. However, if you play another mission, the game should sync your stats again, which will unlock the achievement (mine didn't pop up on the screen like it normally would).
