I've done all the Titanfall achievements on both consoles and with 3 gamertags,
and here is my ultimate solution:

    Important note:
This is my way to play, I played very offensive/rushing towards the enemies.
If you like to chill out at the harvester then of course you need a different
    loadout/titan etc.

Of course 3 good players are needed.
Before we start on the best maps, here are the best loadouts:

    Pilot:
    Smart Pistol (+ Enhanced Targeting)
    RE45
    Arc grenades
    Cloak
    Quick Reload
    Warpfall Transmitter

    Titan:
    Stryder (!)
    Triple Treat + extended mag
    Electric Smoke (aka Titan Fart)
    Big Punch
    Fast Autoloader
    Cluster Missiles

    And now to the maps,
these are the easiest maps to get 2 or 3 stars on:

    Airbase
    Angel City
    Boneyard
Overlook (easiest map for 3 stars)
    Lagoon
    Fracture
    Demeter
    Nexus
    Relic
    Swampland
    War Games
    Haven
    Export
    Dig site
    Zone 18

Happened again! Completed all waves on Angel City. Only one star. This is really pissing me off. I have about 8 maps done now.

    EhrenfriedDid the harvester got damaged?
Did anyone on your team die?

    probably

EhrenfriedI think, but never found real proof that not all maps have the same 2 or 3 Star conditions.
But Angel City is really easy, compared to nexus.

Turrets on the roofs close to the harvester to annoy the foot soldiers
2 titans stay close to the harvester and kill the nuke titans
2 titans rush to kill the mortar titans.

I know. I kept getting useless help. It was good for my MVP challenge, but I've completed it once out of 4 attempts, and it's ridiculous completing it alone isn't enough for 2 stars. I was letting my auto titan do his thing around the harvest, with 3 turrets up on the roof by the ammo crate pointing toward the harvester area, and I was on that roof abusing the hell out of the ammo crate just chucking arc grenades and zapping titans with my charge rifle.

ParadoxReal8What do you mean not Cove!??? Smuggler Cove was the only maps I never die and did easily. For me, it was the best maps ever. Short and easy to go all around fast.

Chameleon314I didn't negative vote but I was hoping to find some kind of info on what requirements for two stars would be. I just read a comment that they aren't all the same but one or two examples would have been nice.

EhrenfriedI don't remember what exactly are the requirements are,
but some maps seemed to have different ones for two/three stars.
(of course the harvester needs to be in good condition)

And my solution here is the offensive way to play,
if you like to be defense and protect the harvester than you have to choose a different Loadout/Titan/Pilot.

el tebyIn my opinion, these are not the easiest maps, there are better choices, such as backwater, training ground, and a couple others, still, this is highly luck based depending on the team you get. If you get a lot of gen 1 players, they are more likely to quit out after losing one wave, so choose higher gen players (or quit out if the lobby is full of gen ones).
